Job Summary
This position is responsible for the operational oversight and program functions for CHOICE’S community based care coordination service lines, to include, but not limited to, the Pathways Model, across a seven-county region. The Manager assures the integrity of daily operations and requirements of the Pathways Model. In this role the Manager will work effectively with a wide range of health partners and community agencies such as the leaders of medical and behavioral health provider organizations, individual health care providers, social service providers, consumers, Tribal members and officials, elected officials, school officials, public health officials, state officials and others involved in efforts to improve the health of our communities. The Manager works collaboratively with various stakeholders and funders to assure program outcome and sustainability goals are achieved.
Major Duties Pathways HUB

  • Maintains prescribed standards of care coordination and ensures services are rendered in conformance with policy and procedural guidelines including, but not limited to, the Pathways-Community HUB certification; Conducts annual review and update of policies and procedures.
  • Monitors key performance indicators to analyze the success of program performance as a whole. Leads data management and outcome measurement analysis including quality assessments for care coordinator and partner agencies. Determines action to improve the quality and standard of care delivered by the program.
  • Takes a comprehensive approach to evaluate and constructively improve current processes and to adhere to community based care coordination, community health worker best practices and/or the Pathways Model. Identifies opportunities for improving efficiency and policy improvement
  • Monitors program performance for the Care Coordination Program, including, but not limited to, documentation performance of individual CHW’s; agencies; or the program in relation to other care coordination projects.
  • Facilitates contracting, onboarding and integration of new partner agencies into the Care Coordination Program including the training of new Community Health Workers and Supervisors for the Pathways model and CarePort technology tools
  • Establish quality assurance metrics that assess documentation compliance. Assures all Pathways are reviewed and for accuracy and completed by CHW’s to ensure compliance with the Pathways Model requirements:
  • Guide process and standards to evaluate and improve referral processes for clients referred to the care continuum; includes use of population health management principles
  • Conduct program audits, including but not limited to chart reviews, development and evaluation of performance measures and outcomes in alignment with Pathways Model standards, AND
  • Ensures scope of practice for non-clinical staff performing care coordination duties via the Washington State Department of Health Community Health Worker Training Program and the Pathways Model
  • Provides teaching, guidance and consultation to partnering agencies community based care coordinators by providing formal education curricula and/or trainings to improve the network scope of practice and promote best practices across the Care Coordination Program.
  • Collaborates with staff, partners, and stakeholders to educate partners on the capabilities of the Care Coordination Program and develops new partnerships that benefit and expand the program.
  • Develop sustainability plan for Community Based Care Coordination program, which may include grant writing to secure funding.

Partner and Stakeholder Engagement

  • Identify stakeholders and partners, effective methods of stakeholder engagement and processes for optimal collaboration related to program(s) and service(s).
  • Communicate and coordinate with key stakeholders and partners, including staff, management, advocacy groups, consumer groups, etc., to meet CHOICE program goals and objectives.
  • Staff workgroups as necessary; manage the coordination of meetings, meeting preparation, and follow-up.
  • Keep abreast of stakeholder activities and present recommendations, insight and analysis to CHOICE Management.
  • Manage stakeholder engagement activities, including drafting regular updates, reports on events, activities and accomplishments for internal and external audiences.
  • Identify training, tools, and material deficiencies within CHOICE programs and services and develop and/or recommend improvements.
  • Attract new stakeholders and funding to support program growth and sustainability.

Contract Compliance & Performance Monitoring

  • Regularly monitor program contracts to ensure organizational compliance and ensure contractual requirements and deliverables are being met.
  • Audit contract compliance of program subcontractors and provide ongoing performance monitoring.
  • Monitor project progress by tracking planned activities, resolving problems and recommending actions.
  • Assure systems and processes are in place to produce program performance reports for Board, funders, contractors and other stakeholders and partners.
  • Write reports meeting reporting obligations and requirements.
  • Assure functionality and progressive improvement of CHOICE’s processes.
  • Identify potential areas of vulnerability within program; develop corrective action plan for resolution of issues and oversee implementation of corrective actions in timely manner.
  • Prepare and maintain records and reports such as budgets, work plans, program documentation and training materials as needed in relation to managed programs and services.
  • Participate in the planning and development of annual program budget; monitor budget and expenditures monthly to ensure maximization of resources while managing expenses.
  • Provide oversight related to implementation for the program, collaboration with data analyst(s), community and tribal liaison, and policy analyst in addition to external resources needed for specific program implementation.
  • Provide related analysis and reporting.

Qualifications Education

  • BA in social work, public health, human services, or similar field, or a comparable combination of education and experience with an emphasis on care coordination.
